Financial Options

Central Park Dental happily accepts Medicare Advantage Plans and most PPO insurance plans. For those without dental insurance, we proudly offer our own in-house dental plan, and we even partner with CareCredit, LendingClub, Alphaeon, and Sunbit as additional financial resources. New Patient Forms

When you reserve your appointment with us, we will email or text you the forms to complete before your appointment. If you prefer to fill out paper forms, please arrive at your appointment 15 minutes early. Completing your forms ahead of time will allow our team to review everything we need to know to make sure you get the best care.
